I have had quite a bit of experience with towing cars and I think the best/cheapest/safest way is to rent a flat bed trailer from uhaul. They are only about $10-20 more than a dolly and are so much safer.
From looking at the photo, I would say it is driver error. The toad vehicle was not secured properly. Just my opinion.

That 65-66 is kind of rough, but I don't think I would call it a "toad" LOL
