the motor has about 98k on it, it's fairly young, is it wrong for me to expect it to go near 200k with good maintenance?

Ford 4.6 Liters can hit 300k if in good hands.

oh 4v swap = swapping the whole engine?, not just heads etc?

any CEL codes? possible misfire with COP or along those lines? could be a # of different things

Drain the oil, if it looks like a milkshake, you've got water in your oil.. and its a head gasket. Don't drive it the way it is.. your gonna need some work done.. and the way its missing and blowing white smoke, water in your combustion chambers will make it so it wont ignite/combust, and if your head gasket is blown, more than likely its not blown for all 8 cylinders, therefore its causing your shaking problem..
