'96 GT has broken 2 TOB sleeves in 5000 miles
I have a '96 with about 150k mi on the motor (completely stock, with new rear main seal and pilot bearing), and the trans was rebuilt around 5000 mi ago. The first time it broke, the shop I took it to just welded it back together and put the transmission back in without checking what I wanted them to do... So that may very well be why it broke the second time, but I'm doubtful because I don't think the sleeve is really under that much stress.
I'm not hard on the transmission at all (I dumped the clutch once, and power shifted into 2nd twice). I rag on it when its in gear occasionally, but I don't shift hard. I don't ride the clutch, and don't stand on it at red lights.
When it goes, it makes the distinct "TOB noise" for about 100 miles, and then starts making an awful clunking racket (presumably when it breaks), with an extreme vibration in the pedal. It also takes the friction plate out, and brakes most of the fingers on the pressure plate.
Is it possible that something is wrong inside the transmission? It shifts fine, and doesn't make much noise at all. I trust the shop that rebuilt it, as well. I'm kind of at a loss, and hesitant to have the transmission torn apart for a new sleeve if it's just going to break again. Any ideas as to what could be the cause?

They welded the sleeve back together. I'm not really sure where exactly it broke, but they told me later that they didn't want to get into taking apart the transmission to actually replace it. Is 4-5 hours right to just remove the bell housing and press in a new sleeve? At that point, would it be worth it to just have a shop tear it all down again? I can pull the trans myself, but I have no intentions of opening it up in my garage...

something sounds funny here the TOB is a bearing and there is really nothing to "weld" back together. And there is no way to remove the TOB without removing the trans. I think your shop is feeding you a load of crap
