here's the first one I found. if you can find a frpp or dealership replacement for close to that price, i would go for it.

Also, any benefit of running a triple hook quadrant vs the double hook?

To answer your first question, I assumed that you were dead set on replacing the stock cable. If it's not noticeably stretched or frayed, I see no issues with reusing it.
Second question, the difference between the two is a patent. I think Steeda had a patent on the double hook quadrant, so to get around it, UPR made a third, useless hook. I forget which one you're supposed to use, but do some research and you should easily figure it out. CliffyK actually did a write up about the triple hook quadrant, and he eventually had problems with it fraying his clutch cable. I, like him, have sanded the edges down and did not notice any fraying when I checked it after about 4-5k miles, so perhaps he missed a spot, or perhaps I'm just lucky and he is not?? I dunno if you would hafta do this with the double hook quadrant or not, but if the edges aren't smooth where the cable rests, I would sand them down as well. Since I haven't had any issues with UPR triple hook quadrant, I would recommend it since its cheaper, just heed the warning to sand down the edges wherever the cable makes contact.

Ok, sounds like a good plan. I saw CliffyK greased the quadrant as well.
In regards to the clutch/flywheel, HDX (clutch) and FRPP (flywheel) seems like the usual suspects for clutch replacement (along with Centerforce Dual Friction, but that's double the cost). How much am I looking at for shop install, $300-400?

Should I look into replacing the clutch fork/other items while I'm in there as well?

May not be a bad idea either ^^.
Found a pretty useful site for this as well:
