Proform electric fan..who has one?

I do, and I un did the wires this last winter, now I don't remeber how to wire it back up...pictures or descriptions would be great.

i tried posting about that same fan the other day. i got replies saying that a ford taurus 3.8L electric fan is better.. so im gonna hit up a junk yard and use that.
