Rebuilt t5 jumps out of 2nd gear.....help!!

I have a 93 cobra spec t5 its a 249 tag number. Its in a 1966 mustang
I rebuilt the trans because it was very difficult to shift into 1st gear....other than that it worked great. Upon inspection the fist gear dogs were worn about half way down and 2nd gear was also pretty bad.
I got the master rebuild kit a new 1st and 2nd gear and a new 1 / 2 slider.
After the rebuild the trans worked flawless for about 3000k miles then every once and a while it would jump out of 2nd... like maybe every 70 shifts....then it was every 30 shifts then every 10 shifts then every other shift. It went from jumping out once to jumping out every other shift in about a month. but had no issues at all for the first 3k.
I took the trans out and inspected it...It looked just like it did after the rebuild. Its still bran new. So I took the output shaft cluster to a shop that I trust and had them inspect it...maybe I screwed something up during the rebuild...They said it looked fine..I had them replace the 2nd gear syncro and the slider tabs and springs....I know I replaced the syncro but Im not sure if I replaced the tabs and springs on the slider.
The shift forks, plastic tabs on the forks and the steel shift plates that fit on the shift rod are all perfect.
Im using a steeda triax shifter and I made sure the stops were set properly...You could pull back pretty hard on the shift lever and still slide a piece of paper between the stop and the shift lever..
I am still waiting for parts to arrive at the shop so I dont know if the new syncro will help yet.
any other ideas?

her's a chart for troubleshooting to the left click on the troubleshooting tab and then scroll down to the table and read the part on jumps out of gear

The shop said they were installed properly...
The counter gear assy was not touched during the rebuild...I left it alone as it looked fine and there is no noticeable end play.. I wonder if that could be the issue....hummm

I'll double check the counter shaft....Maybe I should have added the new bearings for it.
But it looked like a low stress part and I was worried I might make it worse by messing with it and not getting the end play right.

I finally found an actual damaged part!!!!
The trans guts are at my neighbors house so I didnt get to check em over as close as I would of if they were here...I still feel like an idiot for missing it upon first inspection...
The the 1 / 2 shift fork on the 2nd gear side has extensive wear as if the slider was pressing against it the whole time it was in gear....well as it wore down it could not fully engage the gear because it wasnt thick enough....this explains how it kept getting worse.
Now the question is why???
I though I had done a good job of adjusting the stops on the steeda triax shifter maybe not. That slider had to be putting a good deal of force on the fork the burn off the faces of the plastic tabs and then proceed to eat the shift fork...Why would it do this???
