Could be a manufacturer defect it does happen, we just installed one 2 days ago the CF breakin instruction was 500 miles normal driving! this was in a sn95 application, said no full power engagement for atleast 500!

Your particular case dose look like a manf flaw! Give them a call and see what they say!

if you only had the miles you had on it and no foolishness they should warrant a new pressure plate at the least!
