Could be vacuum, could be several things.
electrical or fuel delivery for the stalling. The throttle staying open after you rev it sounds like possible throttle body or throttle cable issue to me. WD-40 your throttle linkage and the throttle flange inside the throttle body. Press the gas several times when it's off, then let it sit for a little. That'll give the WD-40 time to work it's magic. If that doesn't fix it, vacuum is a likely culprit.
IF YOU WANT THE EASY WAY OUT OF VACUUM SYSTEM TROUBLESHOOTING:
Go to a garage and see if they have a smoke machine for vacuum systems. They basically plug into the vacuum system and pump smoke into it. Anything that isn't sealed properly/damage will, obviously, leak smoke. It's going to cost some money, but in my opinion, it's MUCH better than having to do it yourself.
