Since we know you have your 5v reference, check the wire that is supposed to be .99v and do a sweep of the throttle from idle to WOT. Does the voltage sweep cleanly and gradually up to around 4.5 to 5 v at wot? If not its possible you ended up with a faulty tps out of the box, its not uncommon. If it sweeps up ok then I suggest you use a small file or drill bit to lengthen the adjustment slot so you can adjust it up to .99. Ive known a lot of pple that had to make the slot longer to get to the correct idle voltage.
I checked the battery ground, and the main engine ground and they both look fine. I fixed a smaller ground that was broken at the battery, checked all the fuses, and cleaned up almost all the fuse connectors on the fuses in the engine bay. However, it still has the same problem :/. I really have no idea what the problem is. Anyone else have any more suggestions?

If the voltage doesn't change at any throttle position then the computer isn't seeing a return on its reference voltage . If you are checking the reference from the ecm using the wire from the ecm that supplies the tps ground in the plug and your seeing the 5 volt reference, then you have a bad tps. If you check the reference voltage using the engine ground you should see the 5 volts showing that you have reference to the tps, if then you check the ref, at the plug using the tps ground in the plug and you see .00volts then the ground from the ecm is broken someplace .

MAJOR UPDATE: Man I feel like kind of an idiot lol. I've seen a couple posts where guys say there is nothing to installing a tps sensor on a 94-95 Mustang, but that is definitely NOT TRUE. I took the sensor off the throttle body and moved the tabs in the sensor with a screwdriver to see if it would climb up to five volts and there it was! So then I started thinking about the mechanics of the sensor. The tps has to be installed in a position where the throttle blade is going to move the tps tabs in a clockwise position. If you just bolt the tps on there.... it doesn't work! After I installed the tps the correct way, the code went away.
Unfortunately, I broke the new sensor in the process of testing it with a screwdriver lol, but how I see it I saved a diagnostic fee and probably some other BS a mechanic would charge me. I also now found out that I installed the one on my Cobra wrong and that means the tps hasn't been working on that for years. I guess because that is professionally tuned it doesn't make as big of a difference, but I bet it has a lot to do with why it just dies at idle sometimes!
