I can tell you that the #1 thing that kills the clutches right now in the 15s is downshifting at high rpms. example, shift into first gear over 15 mph and you will shear the friction material off of one side of the clutch.
15 mph in 1st gear isnt high rpm's, second shifting like an idiot ruins clutches. however for the op he may have just gotten a defective unit, it happens

Excuse my ignorance, and a quick Google turned nothing up, but what exactly is "second shifting." Do you simply mean the actual shift into second?
Also I regularly engine brake as I see it as a perk of a manual. Usually nothing over the next gear down and gradually slowing down that way. Is that something I should be avoiding?

If you're doing this for performance into a turn, great, enjoy. If you're using the engine too much to actually slow down the car I'm just going to say this: brakes are cheaper than clutches.
