Default Catastrophic Secondary Chain Tensioner Failure

Anyone familiar with this? I was accelerating hard and the car suddenly stalled on me. Tried to turn it over and once it finally started the car started shaking badly. Turns out it was only running on 4 cylinders. The secondary chain tensioner failed and snapped a chain. Here's a pic of the tensioner and cam sproket:

I believe the tensioner failed somehow under high RPMs and then the retainer clip came off and shot up into the chain, got caught in the sprocket and busted the chain. If you look close you can actually see the sprocket marks in the clip and also where the teeth of the sprocket got sheared off.

Anyways, I'm looking for an upgraded timing system so this won't happen again. Has anyone ever seen or heard of this happening before?
