My driver's side broke. The little tabs all broke off at the same time. I found out it is a f'n $60 piece of plastic. Im not paying that so some in here suggested velcro. Yup. I velcro'd it back in...

Came with it. The guy before me put a 99 Cobra front cover on and the back which Im guessing is a 99-04 cover.

If its off at the switch it shouldn't cycle on at all. And the coolant has no bearing on if it comes on or off. It would simply blow hot air if low or empty. FAIK

But with those codes and its behavior it points to a vacuum leak.
