Those aren't Ford diagnostic codes as far as I know. I believe they relate to the CANBUS system in the car. Does your engine, ABS, or traction light(s) flash when it hesitates?

From my experience with multiple exhaust set ups, don't go off-road if you want the best sound. I love my longtubes, O/R ProChamber, and Bassani Race mufflers but many times think it sounded better...

When was the last time you ran a fuel system cleaner? A friend of mien had a serious misfire on his 3v and a can of SeaFoam solved it after the dealership couldn't diagnose.

Many 3v misfires have been solved by SeaFoam. Love the stuff, buy it by the gallon and use it on all the cars I maintain.

I would think that, provided there aren't underlying issues, starting the car and letting it run for 10-15 minutes every 2-3 weeks would be more than sufficient. Might be worth doing the cleaning...

I agree here. If you've been using it daily and allowing the engine to get up to operating temp every time you should be fine. I have seen some people use Tufoil but I have not personally.
