Default Engine/Rev problems

So my clutch (I think it's the clutch) has been being stupid lately and when it engages it seems to catch very hard and it will jerk the car when it does this. So yesterday it did this and it made a different noise when it did it, so I pushed the clutch back in and it stopped and I took off again and for a minute it made a strange noise so I tried to run it through the gear, and when I hit 4k rpms it wont go any higher. I pulled over and tried to investigate the issue but didn't see anything obviously wrong from the outside, except that when it reaches that limit the engine will actually shake back and forth (left-right, driver-passenger motion) and it almost sounds like, hissing? Sorry, doesn't seem like a lot of information to work with but if anyone has any info that might help it would be appreciated.
