p2254 '12 v6 3.7

hey guys getting a code on my wife's 12 3.7 p2254. only mods are cai and exhaust. any ideas what would cause this code? I understand it has something to do with a 02 sensor negative open

If I had to guess, it would be a failed O2 sensor. Does it come on and go off?

it came on yesterday when we were going on about an hour trip and later on that night it didnt come back on and now its not on either.
