I know on my hurst, all I do to shift from 2nd to 3rd is just put my palm on the shifter (I don't touch it with my fingers) and then push up. It automatically goes into 3rd. In fact, if you try to move it over like I had to with the tri ax it goes into 5th about 3/4 of the time, and that is embarassing.
My little bro was bitching about how tight and close the hurst was, and how impossible it was to shift fast, until I told him to quit ham handing the shifter and just barely touch it. Then he liked it.

It does truly sound like a Driver issue, more than a shifter issue..
Most 5-spds default to the very center of the shift pattern, since the bushings, and shift cables tend to be set to center... W/ the car off, put your car in 2nd, and just push it forward, and I can almost guarantee that every time, it'll push DIRECTLY towards 3rd gear...
I've NEVER miss a 2-3, but I'll be damned if I haven't destroyed some shift forks, and syncros on the 1-2...
