Default Clutch Engagement

I just installed a spec stage 2 clutch on my original resurfaced flywheel and installed a TKO 500 transmission behind it. I also replaced the clutch fork, but I used the original pivot bolt. When the clutch pedal is fully released, the fork rests against the back of the bellhousing. I drove it for the first time yesterday, and I noticed the point of disengagement is at the top of the clutch pedal travel. I also noticed slipping when I applied throttle in 1-4th gears.

Is the clutch fully engaged? Is using the factory pivot bolt right? How can I check to ensure full engagement? I realize this is a new clutch, and it might be because it is not broken in, but has anyone else gone through this or something similar, and what did you do about it? I appreciate any help on this since I am at a loss here.
