Any air leaks between the MAF and the TB would cause exactly the problems you describe.

If a visual inspection does not reveal anything use a 3 ft or so length of fuel line attached to propane torch (unlit of course) to direct a flow of propane at various segments of the intake plumbing--if the engine speed increases (or even stabilises) then the propane flow is hear the leak.

Some people use TB cleaner or other flammable liquid sprays, however this is a long way from the safest way to check as the aerosol liquid can puddle up and create a fire. The propane dissipates quickly in air and is far safer.

as an update on mine, I had the throttle body rebuilt. runs great now
