Default Trans output shaft seal

Here is my problem, I have replaced my trans output shaft seal at least four times that I can remember. Once in 08 then again in 2011. I recently had my clutch replaced last fall and had the seal done then. Now last month I brought my car in for a checkup and the damn thing was leaking. So out it comes and a new one goes in.

Is there something else that I am missing? Why can't I keep it from leaking? I have had two theories, one - from the guy who changed out the clutch - is the slip yoke is pitted slightly preventing a tight seal. Second, from the interwebs, is the trans bushing - don't know the correct name - has gone bad and needs to be replaced.

Thoughts? Do I need a new driveshaft or a new bushing? Is anyone else having this issue?
