Sounds like the clutch cable may need to be adjusted or replaced, or the quadrant too. A lot of times the throw out bearing won't be released all the way because of the cable and it won't let it into gear smoothly. I had put in a brand new clutch and it started doing the same thing at about 2000 miles, ended up being a bad clutch, then put another brand new ram clutch in, and what had happened was the factory quadrant was too weak for the clutch and I ended up getting a steeda quadrant with firewall adjuster and OEM cable. Fixed the problem right up. Try pulling up on the clutch pedal until you hear a click or if you have the firewall adjuster, turn it a couple clicks and that should fix it. If not, then it could be bad synchros or something with the tranny. But my guess would have to be either the clutch, or cable/quadrant. The guys on here helped me fix mine so you should get some helpful tips if my reasoning doesn't work.

Is the clutch pedal really heavy to push in?

It sounds as though the clutch is not fully disengaging, how much free play does the pedal have? It should be 1" to 1-3/8"...

Do you have a short throw shifter installed?

There is a TSB referencing reverse problems with the T-45, click here, however the bottom line is that the tranny needs to be rebuilt/replaced...
