How do I work my oil filler cap?

How do I work my oil filler cap?
It may sound silly, but:
My oil filler has a childproof cap, only it works in reverse so that you can't put it on too tight. What I mean is, the part of the cap that you grab rotates seperately from the threaded part that screws into the filler neck. So when I go to screw it on, it doesn't go on very tight, unless I cheat and grab the lower segment of the cap. If I screw it on holding the top of the cap, it seems like it is loose enough to rattle it's way out of the filler neck.
1. Can someone explain what the purpose of this is, and how I should install the cap?
2. While I'm at it, can someone explain what the hose that runs from the filler cap to the intake manifold is for? Crankcase ventilation?

Thanks for the replies, folks. I'm confused, I thought the PCV system was behind the intake manifold, near the firewall. Also, my filler cap doesn't click. Should it?
