sometimes I'll drill a VERY small hole in the throttle plate but mostly do this for S/C or turbo cars

So I thought the mountain dew can IAC block off was a quick fix, and last night, the car ran great with it- no missing or screwed up idle. Now, today it decides to be a pain in the *** again. Idle stays pretty good, but when I drop it into gear, it kills, especially when I shift into reverse.

Any ideas?
