check what the tps is saying. also could be your mass air. if you have the stock mass air look at it and see if the little white(should be) contacts are black or anything. the tps should say .98 or something close.

have you checked codes? i had same problem then discovered that tsb on the 'salt and pepper shaker" connections and when i cleaned them up it fixed the prob. i think the tps is supposed to be around .8

You should try cleaning the IAC motor. That causes alot of idle issues. Look in the t/b and see if it's dirty also.

The first thing that I would do is clean your MAF, 90% of idle problems come from a dirty MAF.

It's pretty much cut and dry. Unplug it, then remove the two bolts holding it to the TB. Take if off and then remove the two screws to seperate the unit. I sprayed carb cleaner in mind and swabbed it with some q-tips. Hosed it out and reinstalled it. I was having some of the same issues as you on a motor with 204,000 miles, but it idles good now. Definately check the TB though. Where mine bolted up, there was some thick black stuff in it. It's definately the easiest thing to check first.

The IAC is the long tube shaped thing similar to a pill bottle that is mounted at an angle right where the intake pipe connects on the TB.
