I'd say that you found your problem! When you take the chip out look at the pins also. They didn't remove the computer did they?

its the chip throwing the PCM code, the other one might be a result of your tune.

you dont have problems, aside from the fact that the tuner didn't tell you to remove the chip when you run codes.
the chip overrides code output. you must remove it to get accurate code outputs, and then replace it when you want.
