If there's slack in the cable then it's time for a replacement. You might get away with adjusting it though.
Oh and Keek89gt, what do you mean you don't use a clutch when shifting. That's your problem right there then, rpm matching puts uneccessary wear on your gears and if you're grinding then try using the clucth that's provided in your car for a reason.
