2nd gear noise

2nd gear noise
when i first shift into 2nd in the morning it makes a clunk noise. dosnt do it in any other gear and only when the car first gets rolling.
also if i get on it too much shifting into 2nd it grinds pretty damn bad, but i assume this is synchro's or maybe clutch adjustment?
