lateral play in clutch fork

lateral play in clutch fork
I've got a TKO-600 that went into my car last year, with a RAM clutch. I have a firewall adjustable cable and a FRPP adjustable quadrant.
I have had this really obnoxious rattle when the car is cool pretty much since the new trans went in. It goes away once the car warms up, or at least gets much quieter.
Well, today, with my car up on a lift, we were looking at the bottom end of the cable and turned the nuts to give some more adjustment room up top at the firewall.
Lo and behold, the clutch fork seems to be really, really loose - it is pretty much resting on the edge of the hole in the bellhousing. There's enough lateral play in it to twist the end up and down and touch the housing on either side. I am guessing this is my rattle, and that the fork is tapping when it's cold because the TOB tends to twist it more with the increased friction of being cold until it heats up. Does this sound like the culprit?
What could cause the fork to have so much lateral play, and how can I address that when I swap in my new motor? I'm thinking it might be prudent to go ahead and replace the fork and TOB while I've got the clutch apart, even though they're only a year old.
FWIW, when the clutch went in, it grabbed very close to the floor and was very heavy. It was a nice feeling. It has since loosened up - it grabs much higher now and isn't as heavy anymore. We adjusted it down as far as we could without risking incomplete engagement, and I still don't like how high off the floor it grabs. Could the fork be bent or something?
