Question Manual trans won't shift when car is running

Today my wife called was driving my mustang and she called me to say that she couldn't get the car out of second gear. She pulled over and when I met up with her, the car was off.

Shifted into all gears with the car off, but once it's started, I can not get it to shift into any gear.

Any ideas? Any help is appreciated!

clutch adjustment? make sure it doesn't start to engage til an inch to 1 1/2" off the floor

how does the clutch pedal feel?

clutch/cable/quadrant/fork, all could be suspect

Feels fine, when the car is off, it just refuses to move out of first gear once I start it. As far as the fork goes, should I be able to move it by hand when I'm under the car? How do I check the fork to see if it's the problem?

Also, should there be a plate covering the area where the fork enters the transmission?...because I see a small threaded hole there, which looks like it would be for a cover plate.
