Default 5.0 Rev Hesitation When Hot, Help?

I'm guessing this is a common issue so sorry if it's been asked a million times.

The car doesn't have this problem when it's warming up but after about 30min when it's completely heated up it starts hesitating pretty hard. There's about a half-second delay between the time i hit the gas to when it revs up. I can hear the air flowing as soon as i hit the pedal and then about a half-second later it kicks in.

It's not making it impossible to drive but man it's irritating. Any idea what could be causing it?

It's an '86 with a '90 5.0. Mostly stock other than cai and exhaust

I would check your vacuum lines, PCV valve and hose. When was this engine put in? And do you have a check engine light on? Also, the EGR and ECT come into mind for me. Hows the thermostat reading?
