Exclamation Clutch misaligment issues

Over the last couple of months of driving my 93 I have had a vibration issue in the drivetrain.... After I had a front ujoint lock up in my driveshaft...and was always looking at the driveshaft as the issue in one way or another. Such as a work yolk and having too much play between it and the tailshaft bushing.

During my tear down for the winter I found this... The rivets on the clutch plate were worn down some bc of the contact with the flywheel as you can see. It is only on the one side.. I would say around a 1/3 of the diameter so its constantly spinning off to the one side, not all over the place.

Ideas? Questions? Thoughts??????

I have yet to open of the trans and check the internals for a possibly bent shaft but it needs to be rebuilt anyway....
