Default Help identifying a Holley

I recently stumbled across a Holley and am trying to figure out what it is.

The numbers on the air horn are:

4118-5 (or S)

0766


I did a quick bing search but haven't found this listed.

Holley list 4118 carburetor

Application - 68 Shelby GT-350 (Replacement- carb)

4V 725cfm

4 Speed

Jets 68 · 78 Pwr Valve 85

Venturi Dia. 1 5/16 · 1 3/8

Throttle Bore - 1 11/16 · 1 11/16
