Turns out the throttle body needed to be replaced... and it was (under warranty). Of course, the PCM was reflashed which is just another pain in my *** (now the speedo is off with the 4.10 gears).
The car was running fine on my way home from the dealership today, until I accelerated onto the freeway. That goddam wrench lit up on my dashboard, and the same thing happened again. Car just failed with no response when stepping on the gas. Stranded on the side of the freeway again, AAA towing came to the rescue, and I just got home after dropping my car off at the dealer for the second time.
I am sooo pissed I can't even see straight. This may be my last American car if this is not resolved quickly.
Comments?? Ideas?

What exactly does that mean-- a bad intake in the same place.
If the dealer reflashed the PCM, doesn't that mean that the stock tune is back in place? Why would the car just up and quit with the stock tune? The only mods I have are the gears and an Airaid CAI that does not require a tune.

They need to outlaw these no tune required CAI's. I don't like them!
You'll need a new tune to correct for you gears. Your old tune won't work because they changed your strategy code. Don't try to reload your 91 tune or it will render the car useless most likely.

Car was at dealer for 2nd time.
You're not going to believe this. I still don't know what to think...
I get a call because they found the problem. There was "some rubber" affecting the MAF sensor. Turns out the rubber was a section of tubing about 1 FOOT LONG, 3/8 inch in diameter, cut on both ends, from God only knows where (STANDARD HEATER HOSE 3/8 is printed on the hose). The dealer asked me where I had the CAI installed because they think that the installer left this odd piece of tubing in the CAI and that it was part of the original packaging (what??!!). I had the CAI installed roughly one year ago!! I think it would have caused a problem much sooner than this, ya think! Also, just a little coincidental that this mysterious piece of rubber tubing was found the day after the throttle body was replaced. Someone at this dealership is f'in crazy and has a serious problem.
The car is running fine now. The speedo was off. I called an experienced mechanic who was a friend of a friend-- told me to use the Predator tuner again to reset the speedo for the 4.10 gear ratio. I did it and put the 91 octane tune on without a problem. Car starts and runs just like it used to. I hope this is the end of this saga.
