Has anyone actually blown a 4.0?

Has anyone actually blown a 4.0?
Ok, we've all read the supersix article about how these motors lose may lose headgaskets at boost, etc...
But has anyone actually lost an engine?
I've read several posts from people running the 100-125 shot, and have heard no problems thus far...
If you have lost a motor, what PSI/size shot of nitrous were your running?
But has anyone actually lost an engine?

I known of one blown motor using nitrous... it was a very extreme case.... something I'll will never run into... Things were being pushed well beyond anything I would do...
