KOEO, KOER, Cylinder Balance Test results

more codes and issues...plz help UPDATED SCROLL DOWN
as some of you know i have been trying to get my car to idle right lately. its been either way to high, or really rough and dying out. I fixed a few vac leaks, cleaned the iac and egr and t/b.
today i got to pull codes. heres the results:
KOEO: 11 pass
KOER: 21, 41, 91, 13..............plz help with any input on what i should look into
Cylinder Balance: 60...states sylinder 6 has a problem.
Now besides the idling issues, and a vac leak that i cant seem to track down, the car has ran good and felt as strong as always. I did however install some new injectors about a week ago, and although it ran fine it didnt feel as strong as usual. The new injectors seemed a little thicker on the top and were fitting tightly into the fuel rails. They didnt seem to go in as deep as the stockers. Could this be an issue with the 6th cylinder?
im not sure what to check anymore...any helps is appreciated.

DTC 21 indicates your temp sensor is not working properly. Which could very well be your problem. That is the first thing I would look into. Check for voltage through the ECT & ACT, make sure they're both connected and getting voltage.
At operating temp the ECT should have roughly .5v (212 degrees) @ 2kOhm say it's 70 outside where you are, with engine cold it should be roughly at 3.00v @ 37kOhm. ACT has same specifications. If either one is out of range substantially, replace it.
Code 13 indicates the KOER test couldn't control RPM, vacuum leaks and improperly set idle can cause this, among other things but those two are most common.
41 & 91 I don't worry about until 21 is fixed.

ok thanks. i took out the coolant temp sensor and its all gunked up and nasty, so ima replace it. i will see if that works.
as far as the idle out of range, i believe your right about that because i can hear a vac leak and the idle is set out of range. i have been trying to reset it, but it always dies out.
i just checked the new injectors i put in. the #7 injector is the one that doesnt fit as snug into the fuel rail. all of the others are in there tight and seem impossible to pull out. but #7 kinda just slips out with a slight pull...would that affect anything?

update...i chamge the coolant temp sensor and that code is gone. i retested and got these codes...koeo came up good at 11 but then gave codes 29, 96. koer codes 12, 41, 91, 13. and the cylinder balance test came out as good 90. so idk whats going on. any other suggestions on what to do next

Ok i really need some input, i cant fig it out. i just adjusted the timing and cleaned out the iac, swapped my egr valve, and adjusted the idle. it finally came down to around 7-800. no more crazy high idle. but when it idles the car feels like it will die but doesnt. the batt charge gauge dips really low as well. then as i give it a lil gas it will be fine. could this be my alternator?
also since ive got it to idle lower, i have pulled even more codes than before.
here they are: koeo 91 koer 22, 29, 63, 66, 87, 96.................wtf? this is gettin really frustrating, any ideas on what i should look at next?
