symptoms:

-shifting is clunky, rough, and notchy unless clutch is all the way to floor from beginning to end of shift, ALL GEARS
-clutch shows no signs of slippage under any conditions whatsoever
-horrific metallic rattling under acceleration, only when in gear with clutch up (released)

i plan on replacing the clutch.meh i don't want to mess with the flywheel because then you have to worry about resurfacing it and such and that can lead to the clutch not breaking in properly, etc.

Cracked flywheel maybe.
Clutch pressure plate bad, that would explain all the rattling

i'll get a video tonight of the noise. it almost sounds like its coming from the rear, but i honestly can't even tell anymore. i've had it for 2 years and checked everything.
