Ok, here is the most common missed item I see when people start getting into heavy mods. Your exhaust, especialy with the long tube and supercharger, is making a TON more heat then your stock exhaust. So much so that the factory plastic clutch line becomes soft and doesnt fully dissengage your clutch.

You may want to replace your slave cylinder as well. there not that expensive, and good insurance.

Also, look for a twist in your in your input shaft, your pushing enough power now that you can actulay warp it.

oh and sometime my shifter is stock in my gear, like already said in another post, cant get my freaking gear out without pulling HARD on the shifter.
Man your trans is so ****ed. If it's getting stuck in gear AND noisy, it's toast. Get a new one ASAP.

I had a T-5 come apart on me onthe freeway. Is was not so cool. One of the bearings came loose and locked up the trans while I was going 70. O.O then I slid to a stop, luckily I managed to get it off of the roadway. Flat spotted my tires too. sucked.

If it only happens once the car is warmed up, or more so when it is hot outside, do the cable first.

If your having a hard time with it feeling like it is stuck in gear, pull it into neutral and pump th clutch pedal 5 or 6 times then hold it in and see if it goes into gear easier.

If it is all the time, look for warping in the input shaft.

Do you know what year your swap kit came from? Mid 08 was when they started shipping our transmissions with beefed up syncros and forks.

can the clutch line be changed without removing everything?
somehow, it happen when the car get hot, when I first leave with it cold, it seem to shift fine, then after 10-15 minutes, it start to get stuck in gear and not properly disengage/engage.. my uncle say it impossible cause last month when he swapped engine they checked my clutch and it was all perfect.

It's a pain in the but cause of where it is, however the external cable can be changed with motor and trans on car.
