My friends1996Mustang GTstumbled at low RPM's, and one day later, his motor threw a few rods. But he narrowed it down to his fuel pump being faulty, so his motor was starved of oil.
If you've replaced all said items, then I would assume its injector related. A COP is also a very common thing to go bad, sometimes water gets in them, and the coils rust, or the rubber boots tear or something.

My COPs would spark fine at idle. It was when they were under load at lower RPM's that they would fail. Running the AC made it worse. I just bit the bullet and bought new ones. I had tried everything else, including a Snap-On scanner.
Best price I found on new COPs were the Accel ones in Brothers Perfomance Ebay store. I think the total was $203.00 with shipping (set of 8). A few of the fine members of thisforum gave me some good feedback on the Accel's. I did not buy them to increase performance, they won't give you any noticable gains on a bone stock car. They were the most cost effective (least expensive) alternative.
