it's more common than people think and usually happens around 100K miles. Mine happened at 121K and was the drivers side. I was just driving through the neighborhood and pop.
The helicoil is when they enlarge the plug hole by drilling it out then they run a tap through it to create threads for the helicoil. Then the helicoil is screwed in.
As long as you check your torque about every oil change you should be fine. I always check them in my wifes 01. If you ever start to hear ticking check them fast.
Yes the above ford fix is correct.
Some aftermarket P&P heads are the older ones and have inserts installed. I would never buy them like that always make sure you get an 03-04 head from the companys. I know DSS only uses the newer heads.

yeah F*** me right i heard ticking for like the last 2 weeks and i told my dad EVERYDAY and hes like "i dont really hear it." "oh there i hear it, its the exhaust" "dont worry about it"

I had right around 70K on my car when #3 on the passenger side decided to play lift off....
it didnt spit out throughmy coilpack, but it did rip up the threads a bit.
Plug actually went back in for a week or so untill I went to check them and it must have ripped the rest of them out putting it back in.
Did my own threaded insert and it worked for about 1K miles before it started banging in plugs.
Final Fix:
Replaced head [8D][8D][&:]

well i got an update my dad stopped by this guys shop today. he drilled through the side and put a pin in to make sure it wouldnt pop out again. also he was doing the helicoil as they were speaking. apparently this guys pretty good with engines so hopefully this wont cost me as much as i thought.
