That depends on how much you want to spend. Hand-shaped pipes, made in an exhaust customization shop would be the best. About 2.5" to 3.0" is good, and as for a muffler, you can skip this (in some states, as the cats silence it enough) or get Flow 40's. They do sound amazing. The loudest you can get is a bullet muffler, from any company that produces racing exhaust.

2.5" is MAX till about 600 RWHP, 2.25 is recomended for 94-98, 2.5" for 99-04 without a power adder

Yes, it is. The concept is the same as getting custom made rear pipes. Side exhaust may cost more, as new brackets must be installed, and hangers placed, not to mention there are things they have avoid under the car. It can be done though.
As for the side skirt question, I'm sure it is possible, if you can get the needed parts individually.
