Default Clutch giving out?

hey guys,

My clutch seems to be acting a little weird lately. When I'm in first and I get the car to start rolling, giving it a sufficient amount of gas, the car will start to feel like it is going to stall with the rattling and such. This happens after I've already started moving and going a couple of mph. Never happened to me before. Is my clutch on its deathbed or am I just driving a bit lazily? Got 12,000 on the clutch with around 30 passes at the track. I'm always having "fun" while I drive as well.

Put it in 4th gear with the brake on let out the clutch. If it dies the clutch is good, If it still runs its shot.

actually the ford repair manual says to put it in 4th, rev the engine to about 3000 rpm then let the clutch out slowly, if it dies clutch is ok, if not the clutch needs to be replaced.
