2006 mustang

I have a 2006 v-6 , 4.0 mustang, i have a hesitation from time to time and usually when i let off gas, engine lopes, changed fuel filter, cleaned throttle body(was not dirty but did any ways).

Did it just start doing it or has it been an ongoing issue for a long time? Any aftermarket mods to your car? When was the last time you had your auto trans flushed?
