Default Car keeps overheating

Well my car keeps overheating, the antifreeze comes out of the cap, no leaks in the pipes anywhere had it pressure tested. I did find out that my cooling fan does not work, well it does when you have the a/c but it makes a clanging sound, like its rubbing against the plastic or somethin. Could it be the fan motor? I know that there are two sensors to turn it on the one for high speed or A/C and the low speed which should come on when the gauge reaches about half correct? Any info on this would be nice. thanks

first,what year is your car? second,have you tried putting a new cap on it?

2003 and yes there is a 16 psi cap on it
