Check out www.2kgt.comHe has a tutorial on exactly what you need and how your suppose to do it. The guys name is Chad and he was very helpful to me while I was doing my swap. He really knows his stuff too.

I have the pedal assembly, clutch fork, clutch, pressure plate, flywheel and driveshaft from a 99 mustang with a t-45, if you need any of the above let me know. I paid just over 200 shipped for the above, and would get rid of it for the same price, only thing besides the above needed to complete the swap is a 5sp, shifter, cable, pilot and t/o bearing, and a set of ***** to do the swap.

If i may i want to add my .02...
I have an auto and i looked into a swap as well.. Let me just say that it is just not worth it. I was getting every single part at a steal and i would have been a little over $1200 into parts. This is with used parts... Do you know what can happen with USED parts?
Other than it being a risk i looked at it as i could use that $1200(+400 for install) for other things. I rode in my buddys stalled/shift kit/geared auto GT and it was AMAZING.
For the money for the swap you can be almost half way too a supercharger or you can do cams etc...
