Default 2 step problem?

What would cause a 2 step to work in neutral but not in 1st with the clutch in?

Wheel speed sensor. And I've heard of a few ppl having issues when they get they're 2step adjustable. Personally mine works fine. Try having ur tuner just set it and make it fixed and not adjustable

Well if it's the things the the abs uses on each wheel, mine are missing and abs/trac control is turned off.

Well that could be your problem right there... the 2 step only works in gear when the vehicle is stationary. (speed sensors read 0mph)

If your car doesnt have wheel speed sensors (WSS) then it cant determine if the car is stopped, thus making your 2-step useless
