Default Plugging up smog holes

there's a huge hole in my passanger side head where exhaust comes out from. from what i was told, it was for the smog application, it was ghetto rigged and kind of plugged up, but it has since stopped working. there is a small bolt pattern next to a pretty big hole and there was a plate bolted to a small bolt covering the larger hole. how can i plug the larger hole without the plate?

its near the back and faces the firewall.

that's the picture of the bolt with the small plate, but it doesnt cover the hole completely so it sounds like an exhaust leak.
