One of two things... you either have bad fluid in there, or you need a new torque converter.
Try this...it works 99% of the time:
Go to a shop and have the tranny flushed. Don't just change the fluid, actualyl have the fluid flushed. (theres a HUGE difference.) After it's flushed, have them put in a bottle of shudder guard.
99% of the time, that will fix the problem. Had it happen with 4 different AODE msutangs (two mine, and other two were friends.) That fixed all four problems. I couldn't believe how dirty the fluid was in my most recent one. It was actually brown!

I work at a ford dealer and have seen a ton of mustangs with this problem. Samseed 101 is right, flushing fixes it 99% of the time but if it doesn't you'll need a new converter. Good time to upgrade if that's the case.
