Default Reverse Grinding

Every stinkin time i start up the car and put it into reverse the dog teeth dont engage and it grinds. why? in the manual it says how you have to go all the way left and reverse as a safety feature. but i do that an it still grinds. also sometimes it seems like the shifter pops out of gears. not during a gear but like ill shift and it will pop out than i just rev it. embarassing .

ok i kind of figured that. ok i know the sound isnt the gears grinding because that isnt possible. its the dogteeth trying to engage. do those ever get messed up when it grinds?
