Default Rocker nut question

Hey, So my last post I said there was a knock and ticking. So I took off the valve covers and found both nuts on the rocker arms were loose. So I tightened them up an started the engine. Wow...it ran so well for 2 minutes. Then the ticking came back. I took off the valve covers to find that one of the rocker nuts (the one that was really loose last time) was loose again!!

How do I stop the nut from backing off?

I took a look at some of the other nuts, and I think that someone took a punch and a hammer to them. It that what you are supposed to do? After you set the valves, use a punch and hammer?

the nut needs to be replaced. They are made to keep from backing up, but when they get old, they dont work that well anymore.
