Default keep losing alternator belts

My alternator keeps coming off. (obviously I have a v-belt setup).
I tried to move the alternator out a little more by inserting washers behind the bracket bolts.
The alternator pulley now seems lined up with the crank pulley but I'm still losing alternator belts for some reason.
Has any else had this problem or know how I can fix this problem?

make sure you don't have a bent pulley or a bad balancer

The pulley isn't bent..checked that. As far as a bad balancer goes, I don't think it could be that because I don't have any vibrations or anything like that.
I was thinking maybe there is a deeper crank pulley or something I could get........
I don't know but I'm tired of losing belts....and I always know when it xomes off because the voltmeter drops from 14 down to 12

they make deep groove alternator pulleys, check out Summit or Jegs for them and use a long straight edge along the crank pulley and alternator pulleys to make sure they are properly aligned and adjust the spacers accordingly to get them dead on aligned.
