The 3 that say lack of upstream o2 sensor leads me to think you didn't have them turned off in your tune, or don't have mil eliminators. I'd replace the front two as well, that should take care of the o2 codes

for the other two,

Sorry I'm on my blackberry so I don't see everything! Is the car not driven often, or have you had to jump start recently? I've been through the situation before and it takes a while for those codes to reset more than a week! Good luck! If its a dd you might need to replace o2 sensors and fuel filter!

Go with what Synsyter said and make sure that the rear o2s are turned off in the tune.

with all the aftermarket intakes running around here, I'm honestly amazed at how often a dirty maf is the culprit, it has become my cure-all tonic. lol

Yeah dirty MAF's, or Ryan likes to point out loose gas caps which is almost NEVER the issue. Must be having a lazy day or days.
